Licensure Application
The application package is not available online. Please send your mailing address to albdpsychology@psychology.alabama.gov and an application package will be mailed to you.

License Verification
Please provide your fax number and email your request to albdpsychology@psychology.alabama.gov

License Renewal
Renewal of your license should be completed prior to October 16, 2011. Your renewal form and check in the amount of $150.00 must be mailed to the Board office and your continuing education requirement met in order for your license to be renewed.
